  1. Download and fill out both credit applications (PDF). Our staff will assist you in deciding which creditor to pursue once your application has been received.
  2. Please sign both applications.
  3. A valid Driver License or Identification number and expiration date is required by GE Money.
  4. A Debit or Credit Card Issuer and expiration date will need to be provided for GE Money.
  5. Mail your completed credit application to:
